This document must be read in full and signed by a parent or legal guardian before any athlete is permitted to access or begin the SP60 training program.
SP60 is an 8-week youth baseball and softball performance training program developed by Standard Performance (SP) for athletes ages 10 through 14. The program includes resistance training, plyometric training, arm care exercises, weighted implement throwing (including regulation-weight and over-weight baseballs), medicine ball work, and throwing velocity development. The program is delivered as a structured programming guide to coaches, parents, and athletes. Standard Performance does not directly supervise athletes at training sessions unless explicitly arranged as part of a paid on-site coaching agreement.
I understand and acknowledge that participation in the SP60 program involves physical training activities that carry inherent risks of injury, including but not limited to:
Muscle strains, ligament sprains, and joint injuries
Arm, shoulder, and elbow injuries resulting from throwing or resistance training
Growth plate stress in developing youth athletes
Fatigue-related injuries resulting from overuse or inadequate recovery
Impact injuries from plyometric and jump training
Cardiovascular exertion
Any other injury associated with athletic training activity
I understand that these risks exist regardless of how carefully the program is administered, and that no training program can eliminate risk entirely. I voluntarily accept these risks on behalf of my athlete.
I have been informed of and understand the following specific risk factors within the SP60 program:
3a — Weighted and
Over-Weight Baseball UseSP60 Phases 3 and 4 incorporate throwing with baseballs heavier than regulation weight (6-10+ oz depending on age group, compared to a standard 5-5.25 oz baseball). Research on weighted ball programs in youth athletes is limited, and some studies in older athlete populations suggest an increased rate of arm complaints in pitchers using weighted ball programs. I understand this risk and consent to my athlete participating in the weighted implement throwing portions of the program.
3b — Mechanics
PrerequisiteWeighted ball training is only appropriate for athletes who demonstrate sound, efficient throwing mechanics. I understand that my athlete's coach or program administrator is responsible for evaluating throwing mechanics before weighted ball work begins, and that my athlete should NOT advance to weighted ball phases if significant mechanical deficiencies are present. I acknowledge that bypassing this prerequisite increases injury risk and releases Standard Performance from any related liability.
3c — Fatigue and
Arm PainTraining while fatigued significantly increases injury risk. If my athlete reports arm pain, unusual soreness, or general fatigue during the program, they must stop immediately and not resume until evaluated by a qualified sports medicine professional. I understand that SP60 is not a supervised clinical program and that SP cannot monitor individual athlete fatigue levels.
3d — Plyometric and
Jump TrainingBox jumps and plyometric exercises carry risk of lower-body injury, including ankle sprains, knee injuries, and falls. Box heights are provided as guidelines only. Landing quality — not height — determines whether an athlete should advance. Athletes must demonstrate controlled, quiet landings before increasing box height.
3e — Resistance
Training LoadsKettlebell and resistance band progressions are provided as guidelines for average athletes in each age group. Loads should be reduced if an athlete cannot complete reps with proper form and posture. Forcing loads beyond an athlete's current capacity increases injury risk.
3f — J-
Band / Resistance Band Arm CareBand resistance levels in the program are guidelines, not requirements. Athletes should not advance to heavier resistance bands unless they can complete all sets with full range of motion, zero pain, and no compensation patterns. This is an athlete-by-athlete determination.
By signing this form, I attest that:
My athlete has been cleared for participation in athletic training activities by a licensed physician within the past 12 months, or has no known medical condition that would contraindicate participation in a structured youth training program.
My athlete has no current injuries to the arm, shoulder, elbow, or lower body that have not been evaluated and cleared by a sports medicine professional.
My athlete has not been advised by a physician to restrict throwing, lifting, or high-intensity physical activity.
I will inform the program administrator immediately if my athlete's health status changes during the program.
I understand that Standard Performance is not a medical provider and cannot evaluate, diagnose, or treat injuries. SP60 is a training program, not a medical or rehabilitative service.
I understand that a well-executed SP60 program may produce an average gain of 1-4 MPH in throwing velocity. Individual results vary. Some athletes will see gains above or below this range, and some athletes may not see measurable velocity gains depending on individual factors including age, development stage, prior training history, physical maturity, and commitment to the program. Standard Performance does not guarantee specific performance outcomes.
To the fullest extent permitted by law, I, the undersigned parent or legal guardian, on behalf of myself and my athlete, hereby release, waive, and discharge Standard Performance, its founders, coaches, employees, contractors, agents, and affiliates (collectively, "SP") from any and all claims, demands, causes of action, and liability arising from my athlete's participation in the SP60 program, including claims arising from the ordinary negligence of SP.
This release covers all injuries, damages, losses, and expenses, including medical expenses, resulting from participation in the SP60 program.
This release does not apply to claims arising from gross negligence, reckless misconduct, or intentional acts by SP.
I agree to indemnify and hold harmless Standard Performance from any claims, demands, or causes of action brought by third parties arising out of or in connection with my athlete's participation in SP60, including any failure by the athlete or program administrator to follow SP60 guidelines.
In the event of an emergency involving my athlete during SP60 training, I authorize any SP coach or program administrator present to contact emergency medical services on behalf of my athlete. I understand that SP personnel are not responsible for the cost of emergency or medical care.
I confirm that I have received or been directed to review the SP60 Program Rules document and understand the program structure, phase guidelines, age-group progressions, and safety requirements contained within it. I agree to follow these guidelines and to communicate them to my athlete.
